بهبود پیامد عملکردی بیماران مبتلا به سکته مغزی ایسکمیک با تجویز زودهنگام تیروفیبان پس از ترومبولیز وریدی

Main Title:Improved Functional Outcomes with Early Tirofiban after Intravenous Thrombolysis in Acute Ischemic Stroke
The results of the phase 3 ASSET-IT trial, published in The New England Journal of Medicine in 2025, provided new evidence regarding the potential role of early tirofiban administration after intravenous thrombolysis in patients with acute ischemic stroke. The study investigated whether adding a potent antiplatelet agent to thrombolytic therapy could improve functional outcomes, potentially by reducing early vascular reocclusion.
This multicenter, double-blind, randomized, placebo-controlled trial was conducted at 38 centers in China and enrolled 832 patients with acute noncardioembolic ischemic stroke. Eligible patients presented within 4.5 hours after symptom onset, were candidates for intravenous thrombolysis, and were not eligible for endovascular thrombectomy. A total of 414 patients were assigned to receive tirofiban and 418 to receive placebo.
Participants received a 24-hour intravenous infusion of tirofiban or placebo within 60 minutes after initiation of intravenous thrombolysis. Alteplase was used in 75% of patients and tenecteplase in 25%. The primary efficacy outcome was an excellent functional outcome at 90 days, defined as a score of 0 or 1 on the modified Rankin Scale (mRS).
At 90 days, 65.9% of patients in the tirofiban group achieved an mRS score of 0 or 1, compared with 54.9% in the placebo group. The probability of achieving an excellent functional outcome was therefore significantly higher with tirofiban (Risk Ratio, 1.20; 95% CI, 1.07–1.34; P=0.001). These findings suggest that early tirofiban administration after thrombolysis may improve the likelihood of favorable functional recovery in selected patients with acute ischemic stroke.
Safety outcomes remained an important consideration. Symptomatic intracranial hemorrhage within 36 hours occurred in 1.7% of patients receiving tirofiban and in none of the patients receiving placebo. Ninety-day mortality was similar between the groups, occurring in 4.1% of patients in the tirofiban group and 3.8% in the placebo group. Although the absolute rate of symptomatic intracranial hemorrhage was low, its occurrence in the tirofiban group highlights the need for careful assessment of the balance between potential benefit and bleeding risk.
The ASSET-IT trial suggests that early tirofiban administration after intravenous thrombolysis may increase the likelihood of excellent functional recovery in selected patients with acute ischemic stroke. However, these findings should be interpreted in the context of the study population, eligibility criteria, and observed bleeding risk. Further studies in independent and more diverse populations will be important for determining the precise role of tirofiban as an adjunctive treatment after intravenous thrombolysis.
